What this website collects
The reservation form asks for a name, a phone number, an optional email address, the type of visit you want and whatever you choose to write in the notes box. Nothing else is requested and no account is created. If you arrive from the home page action bar, the sentence you typed there is copied into the notes box so you do not have to type it twice.
Standard web-server logs record an abbreviated IP address, the page requested and the time of the request. Those logs exist to keep the site online and to spot abuse.
Why we hold it
- To call you back and agree an appointment time.
- To run an insurance benefits check before you arrive, when you have asked us to.
- To keep a short record of appointment requests so a missed call can be traced.
Please do not send clinical detail by form
A short line about what hurts is helpful and expected. Diagnoses, medication lists, scan reports and anything else you would consider a medical record should wait until you are in the building or on the phone, where they can be stored properly.
Who sees it
Requests are read by the front desk and the treating doctor. We do not sell, rent or trade contact details, and we do not pass them to marketing companies. The only third parties involved are our website host, which stores the site files, and our email provider, which delivers the request to us. Both are bound by their own confidentiality terms.
How long it is kept
An appointment request that does not turn into a visit is deleted within twelve months. If you become a patient, the request is folded into your clinical file and kept for the period Georgia law requires of chiropractic records.
Cookies and analytics
This site sets no advertising cookies and runs no third-party tracking pixels. Fonts are loaded from Google Fonts, which means your browser contacts that service to fetch the typeface files.
Your choices
You can ask what we hold about you, ask for it to be corrected, or ask us to delete a request that has not become a clinical record. Write to Jeremy@hesschiro.live or call (943) 244-7380 and we will answer within one working week.
Children
Children are treated at the center, but the website form is intended for a parent or guardian to complete. We do not knowingly collect details submitted by a child on their own.
Changes to this page
If the way this site handles information changes, the date at the top of this page changes with it. Material changes will also be mentioned at the desk on your next visit.
